We accept all insurance except any Medicaid product, and we are not currently accepting new Medicare patients.
Patients with Insurance
If you have insurance, it is your responsibility to know how much your insurance will cover. We accept hundreds of different insurance plans, and we cannot track the details of each plan. It is you who have the contract with your insurance company, not us. And so while we bill them as a courtesy to you, ultimately the cost of services provided by us is your responsibility, and it is your responsibility to know how much your plan will pay.
Please note: You must call your insurance in advance of your appointment to see what percentage of the particular service they will cover.
Bring your insurance card to your appointment. Co-pay and deductible are due at time of service.
We accept cash, check, and credit and debit cards (VISA, MasterCard, but not American Express). On the rare occasion that we cannot calculate what you owe on the day of service, we will send you a bill after we receive your estimation of benefits. Bills are due immediately, and payment may be made by mail, in person, over the phone, or online.
Late fees are assessed on an increasing basis after 30/60/90 days.
Patients without Insurance (self-pay)
We happily accept patients without insurance on a self-pay basis. You can pay for the visit out of pocket at the time of your visit. We accept cash, check, and credit and debit cards (VISA, MasterCard, but not American Express).

- Additional visits $75-$250
